I just created a teacher survey form and put it on a google doc and shared with 6 teachers. They are submitting their responses online and it automatically creates a spreadsheet to show the questions and responses.
The interesting thing to me was I am able to do this for documentation required by College Board for a student to apply for extended time on their SAT. So then I print it out and fax it to the College Board and it shows that the student really is using her accommodations and needs them and the teachers acknowledge that.
I know you are all much further along on the wiki and google apps, but there was an example of an application I could use as a counselor to help someone.
